ICHOM Heart Failure and Stroke Standard Sets example

This workshop will bring together value-based healthcare (VBHC) experts and patients in an interactive session to discuss and deliberate on the importance of the outcomes measurement to organisations and most importantly patients. A panel of experts will share their expertise and provide guidance on how to develop tools for outcomes measurement, data analysis and reporting, communication and use of those outcomes for identifying and adopting best practices and opportunities for care improvement.


Topics to be covered:

  • What is patient-reported outcomes measurement and why do you need to do it to provide value-based care
  • Identifying the current best means of collecting patient-reported outcomes (timing, place, language, use of existing or new technology etc)
  • How to verify the accuracy of your data to ensure it is of high quality
  • How to report your data in the most meaningful way, depending on the goal the organisation wants to achieve
  • Importance of looking at data, outcomes and costs in real time to ensure improvements in care
  • How to use measured outcomes for identifying best practices and opportunities for improvement

The webinar will provide an insightful session of how care providers from various geographies measure patient-reported outcomes and use the outcomes data to identify best practices and opportunities for patient care improvement. You will learn from patients themselves about their experience of reporting the outcomes and their involvement in determining a course of treatment based on the collected outcomes.
